Physicals for student athletes in the 2011-2012 school year must occur after June 1, 2011, per PIAA. Re-certification of a physical from before that date is no longer possible.

IMPORTANT NOTE: Physicals for sports in the 2011-2012 school year must be dated AFTER June 1, 2011. If your child is going to be participating in a sport, a physical examination is required BEFORE they may practice. Have your child's doctor complete the Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) form (Pages 1-4) at the exam.

Parents may fill out the Re-Certification Form if your child is participating in a second or third sport during the school year.

The Re-Certification form should be used for students who previously completed the CIPPE form but then suffered an injury that caused them to miss more than 25% of the season, or students who missed school for more than ten days due to illness, injury or surgery.

Pursuant to the Weight Control Program adopted by PIAA, prior to the participation by any student in interscholastic wrestling, the Minimum Wrestling Weight (MWW) at which the student may wrestle during the season must be 1) certified to by an Authorized Medical Examiner (AME) and 2) established NO EARLIER THAN six weeks prior to the first Regular Season Contest day of the wrestling season and NO LATER THAN the Monday preceding the first Regular Season Contest day of the wrestling season
