REQUIRED NEW EMPLOYEE PAPERWORK

- Earned Income Tax form
- Health Form with a completed TB test (completed form due to HR Office within 30 days of employment)
- Prior School Employment - New/Existing Employee Form
- Employment Eligibility Verification (I-9) Complete Section 1 and provide supporting documents as listed on the back of form. We must view original documents as follows:
- one document from List A OR
- one document from List B AND one from List C
- W-4 Form
Clearances Required:
- Provide validated Criminal Record Check
Please go directly to the PA State Police Website to submit the form electronically: https://epatch.state.pa.us/Home.jsp
- AND Child Abuse History Clearance Form
- AND FBI Check
- AND ACT 24
IF you don't have current clearances forms (not more than one year from the issue date), you must sign an Affidavit and submit to Central Bucks with copies of your clearance applications while you wait for your original clearances to come from the state.
